CRIMINAL HISTORY LETTERS FOR INDIVIDUALS AND BACKGROUND CHECKS ON OTHERS
In order to obtain a criminal history letter or a background check letter you are required to fully complete and sign the Records Request Form and pay the appropriate fee. If you would like to pre-fill out the Records Request form prior to coming into the office to save time please click on the Records Request form under Quick Links. Please be sure that you include your full name, including a middle name and date of birth if you are obtaining a criminal history letter on yourself. If you are requesting a background check letter on another individual you must have their full name and date of birth so that the Sheriff’s Office is assured that we are releasing information on the correct individual. If you cannot provide verifiable information on the individual that you are requesting a background check letter on we will NOT produce a letter.
WE WILL ONLY PRODUCE A CRIMINAL HISTORY LETTER FOR A REQUESTING PARTY IF THEY PHYSICALLY COME IN TO OUR OFFICE AND PRESENT THEIR OWN, GOVERNMENT ISSUED PHOTO ID (e.g. driver’s license or state ID).
The only exception is when the requesting party is unable to physically come into the Douglas County Sheriff’s Office and has completed a signed and notarized Douglas County Sheriff’s Office/Records Unit Criminal History Request Form, a separate form from the Records Request form that states in detail an approved reason for not being able to physically come into the Sheriff’s Office. Please call (303-660-7545) and confirm that you are eligible to utilize this form PRIOR to submitting it to the Records Unit, due to the fact that if you are in fact not eligible it will be returned to you requesting that you come into our office. It is NOT our intent to delay these services, only to provide accurate and timely information in a professional manner. You are also required to submit a legible photocopy of your government issued photo ID (e.g. driver’s license or state ID), include a self-addressed stamped envelope and prepayment for the letter. Example of “approved reason” for not being able to physically come into the Sheriff’s Office is that the requesting party is out of the Country, Out of County___ miles from your location (e.g. outside the metro area), or medically/physically unable to travel.
